Raffi Ceylan was alive on 11 July 2016.
Raffi Ceylan did not die until 16 July 2016.
The autopsy itself was not conducted until 18 July 2016.
The document therefore constitutes, on its face, a signed medical forensic opinion about a post-mortem examination of a person who was not yet dead, conducted on a date that had not yet occurred.

This is not a scrivener's error. It is not a clerical typo. It is the unambiguous, machine-generated date stamp of an electronic-signature system recording the moment a forensic pathologist affixed his professional credential to a medical-legal document opining on the cause of death of a man who was on that date still walking, breathing, and corresponding with his family.
Per R. v. Trochym, 2007 SCC 6, ¶32, the Supreme Court of Canada held that where expert evidence contains temporal impossibilities or logical contradictions, the trier of fact must draw adverse inferences regarding the reliability and authenticity of that evidence. A signature pre-dating the event the signature certifies is the paradigm case of such impossibility.
The resulting document is a false document within the meaning of Criminal Code s. 366 (making a false document), its subsequent use constitutes an offence under s. 368 (use of forged document), and its employment to suppress a criminal investigation constitutes obstruction of justice under s. 139(2) and breach of trust by public officer under s. 122.
The temporal-impossibility forgery does not stand alone. It is the keystone of a six-document pattern of systematic forensic suppression in the investigation of Raffi Ceylan's death:

Vomiting is a primary symptom of poisoning by strychnine, ricin, and arsenic. Lucy Ceylan reported to the Regional Supervising Coroner that in the three days before his death, Raffi Ceylan suffered severe headaches, ocular twitching, and a disoriented daze — the classic prodrome of organic-poison intoxication. None of this was pursued.
